Regional session for individuals interested in supporting Ontario Health Teams

RISE will host six regional sessions for individuals interested in supporting Ontario Health Teams (OHTs). RISE welcomes the active participation of individuals who can bring different types of expertise to supporting OHTs, including:

  1. researchers who bring research knowledge
  2. patient partners, family members and caregivers who bring lived experience
  3. health-system partners who bring experiential and tacit knowledge
  4. health-system partners who bring contextual knowledge (e.g., care in the north)
  5. health-system partners who bring much of the above plus delivery-mechanism capacity (e.g., coaching and communication infrastructure)

Each session will cover the same content and will last 60 minutes. Individuals can choose to participate in person or via Webex. A Webex recording of the ‘core content,’ as well as a Webex recording of each of the six question-and-answer sessions, will be posted on the RISE website.

The draft agenda for the session is as follows:

  1. introductions and welcome (5 minutes)
  2. brief presentations on five topics (25 minutes):
    • OHTs 101, including the OHT ‘building blocks’ (and links to the OHT full application form) and areas of strength and opportunities for improvement emerging from readiness assessments
    • OHT Central Program of Supports, including RISE and its website
    • leadership and governance, including leadership infrastructure and work plans, collaborative governance, and primary-care leadership and engagement
    • population-health management and how data analytics and research evidence can help
    • rapid learning and improvement and how a community of practice can help
  3. questions and answers (30 minutes)

Details for the downtown Toronto session
