By-invitation only regional session for teams invited to full application

RISE will host six regional sessions for teams invited to complete an Ontario Health Team (OHT) Full Application Form. Each session will cover the same content and will last 90 minutes. Team members can choose to participate in person or via Webex. A Webex recording of the ‘core content,’ as well as a Webex recording of each of the six question-and-answer sessions, will be posted on the RISE website.

The draft agenda for the session is as follows:

  1. introductions and welcome (10 minutes)
  2. brief presentations on five topics (40 minutes):
    • OHTs 101, including the OHT ‘building blocks’ (and links to the OHT full application form) and areas of strength and opportunities for improvement emerging from readiness assessments
    • leadership and governance, including leadership infrastructure and work plans, collaborative governance, and primary-care leadership and engagement
    • population-health management and how data analytics and research evidence can help
    • rapid learning and improvement and how a community of practice can help
    • OHT Central Program of Supports, including RISE and its website
  3. questions and answers (40 minutes).

Details for the downtown Toronto session
